Effective Date
This Agreement becomes effective on the date signed and remains in effect according to the selected membership or commitment term.
Membership and Billing
Membership is offered on a recurring basis or for a defined commitment term, as selected at enrollment. Fees, billing frequency, and start date are agreed upon at signup. Membership fees are billed in advance and processed automatically using the payment method on file.
Payment Authorization
The Member authorizes Lewisville Boxing Team to charge the payment method on file for all membership fees and any outstanding balances. The Member is responsible for maintaining valid and current payment information at all times.
Card on File Requirement
A valid payment method must be maintained on file at all times. Removal or failure of a payment method may result in immediate suspension of training access until updated.
Failed Payments and Account Standing
If a payment fails, the system may retry the charge. Accounts with outstanding balances may have training access paused until the balance is resolved. Continued nonpayment may result in account suspension or termination.
No Pay, No Train
Active participation requires an account in good standing. Members with unpaid balances or without a valid payment method on file are not permitted to train until their account is current.
Month to Month Cancellation Policy
Month to month memberships require a minimum of 10 days notice prior to the next billing date for cancellation. If notice is not provided within this timeframe, the next billing cycle will be charged and the membership will remain active through that period.
Commitment Based Memberships
Commitment based memberships, including 6 month or 12 month agreements, are not subject to the month to month cancellation policy. These memberships remain in effect for the full term selected at enrollment and automatically conclude at the end of the commitment period unless otherwise extended.
Early Termination
Members enrolled in a commitment based membership who wish to terminate early agree to either pay the remaining balance in full or pay an early termination fee equal to 50 percent of the remaining contract value.
Outstanding Balances
All outstanding balances must be resolved before a membership is canceled, completed, or reactivated. Lewisville Boxing Team reserves the right to pursue collection of unpaid balances.
Freeze Policy
Membership freezes must be requested and approved in advance and are limited to a maximum of two months per contract term. Approved freeze time will be added to the end of the agreement. Missed or declined payments do not constitute a freeze.
Accrual of Charges
Unless a freeze has been requested and approved, the membership remains active and all scheduled charges continue to accrue according to the agreed billing schedule.
Reactivation
Accounts with outstanding balances must be brought current before training access is restored.
Assumption of Risk
The Member understands that boxing, fitness training, and related activities involve inherent risks, including but not limited to serious injury, permanent disability, or death. The Member voluntarily assumes all risks associated with participation.
Release of Liability
The Member releases and discharges Lewisville Boxing Team, its owners, coaches, staff, and affiliates from any and all claims, liabilities, or damages arising from participation in any activities, including injury or loss, except where prohibited by law.
Medical Responsibility
The Member confirms they are physically able to participate in training activities and assumes full responsibility for their health and medical condition. The Member agrees to seek medical clearance if necessary.
Conduct and Safety
Members agree to follow all gym rules, safety instructions, and coaching guidance. Lewisville Boxing Team reserves the right to terminate membership for unsafe or disruptive behavior.
Photography and Media Release
The Member grants permission for Lewisville Boxing Team to use photos or videos taken during training for promotional purposes unless otherwise requested in writing.
Policy Updates
Lewisville Boxing Team reserves the right to update membership terms and policies as needed. Continued participation constitutes acceptance of current terms.
Member Acknowledgment
By signing below, the Member acknowledges that they have read, understood, and agree to all terms outlined in this Agreement.
